A Family Guide to Understanding Evaluation/Re-Evaluation Reports

Statewide Trainings

This session will guide families in understanding the updated Annotated Evaluation/Re-Evaluation (annotated) forms. Families will gain an understanding of how to use forms and how to provide important information about their child’s strengths and needs as part of the evaluations team. The presentation will guide participants through these forms to better understand how each section of the evaluation forms assist parents and others on the IEP teams in developing goals. The presentation will also better prepare the IEP teams in identifying which supplementary aides and services would benefit the student. There will also be several handouts.
